Phonological Processing Disorder

A phonological processing disorder impacts a child’s ability to organize, represent, and use speech sounds within a language system. The primary difficulty is not how an individual sound is produced, but how sounds function and pattern together.

Phonological disorders are frequently misidentified as articulation disorders. While the two can co-occur, they are clinically distinct and require different approaches to assessment, goal selection, and treatment.

What are phonological processes?

To understand phonological processing disorders, it’s essential to understand phonological processes.

Phonological processes are rule-based simplifications children use as they develop speech. Producing adult-like speech requires precise coordination of respiration, phonation, and articulation. As a result, children naturally simplify speech to reduce motor and linguistic load. These patterns are typical and expected during development.

Example:
Final consonant deletion occurs when a child omits the final consonant of a word (e.g., “wa” for “water”). This process is developmentally appropriate in young children and typically resolves by approximately 3 years of age.

A phonological disorder is present when these processes persist beyond expected ages, occur with unusual frequency, or significantly impact intelligibility.

What causes a phonological processing disorder?

In most cases, there is no single identifiable cause.

Phonological disorders affect about 10% of preschool children (National Institute on Deafness and Other Communication Disorders, 1994).

Current research suggests that children with a phonological processing disorder often present with co-occurring expressive and/or receptive language delays, indicating broader language system involvement rather than isolated speech sound difficulty.

Articulation Disorder vs. Phonological Processing Disorder

Articulation and phonological disorders can look similar on the surface, which is why misdiagnosis is common. However, key features help differentiate the two.

Common Characteristics of Phonological Processing Disorders

  • Significantly reduced intelligibility
  • Consistent, rule-based error patterns
  • Atypical or unusual sound substitutions or omissions
  • Multiple sound errors affecting the phonological system

This list is not exhaustive. A comprehensive speech sound evaluation—including pattern analysis—is required for accurate diagnosis.

Treatment Overview

The primary goal of treatment for a phonological processing disorder is improved intelligibility through system-level change.

To achieve this, therapy must address both:

  • Accurate sound production, and
  • Correct organization of sounds within the language system

Treatment selection should be driven by error patterns, stimulability, and overall system complexity—not isolated sound errors.

Sensory-Motor Approach

A sensory-motor approach targets both speech perception and speech production.

Auditory Bombardment

Auditory bombardment supports phonological learning by providing high-frequency exposure to target sounds or patterns. This may include word lists, books, or play-based activities rich in target contrasts.

Motor Speech Practice

Production practice follows a structured hierarchy:

  • Isolation
  • Syllables
  • Words (initial, medial, final)
  • Connected speech

Feedback (visual, verbal, tactile) is provided initially and systematically faded to promote independence.

Cycles Approach

The Cycles Approach was created by Hodson & Paden and is widely used with children who have phonological processing disorders.

This approach:

  • Combines auditory bombardment with production practice
  • Targets patterns rather than individual sounds
  • Cycles through multiple error patterns instead of waiting for mastery before moving on

The goal is gradual system-wide improvement rather than rapid mastery of isolated targets.

Minimal Pairs

Minimal pairs are pairs of words that differ by a single phoneme and contrast meaning (e.g., “tag” vs. “bag”).

Minimal pairs therapy targets:

  • Phonological awareness
  • Sound contrasts
  • Meaning differences resulting from sound errors

Children must perceive and/or produce the contrast correctly to demonstrate phonological learning.

Take-Away Messages

Based on current research and clinical practice:

  • Treating both early- and later-developing sounds can be effective; however, targeting later-developing sounds first may lead to broader generalization
  • Targeting non-stimulable sounds may be more efficient and result in greater system-wide change than starting with stimulable sounds
  • Treatment decisions should always be individualized, data-driven, and responsive to the child’s phonological system
